On August 16, 2007, a Notice of Violation (NOV) and Proposed Imposition of Civil Penalty in the amount of $3,250 was issued for a a Severity Level III violation of 10 CFR 30.34(i) involving the failure to maintain a minimum of two independent physical controls that formed tangible barriers to secure a portable gauge from unauthorized removal during a period when the portable gauge was not under the control and constant surveillance of the licensee, which may have contributed to the theft of the gauge. Specifically, a portable nuclear gauge was stolen from an unattended licensee vehicle that was parked at an employee residence. While the gauge was unattended, the licensee had only a single tangible barrier in place to prevent the theft.
